European Social Fund Report Preparation Overseen by the Commission

The Community Nurse Plus project, implemented in Rhineland-Palatinate in 2015, is a preventive, health-promoting, and loneliness-reducing initiative that has expanded significantly since its inception. Originally a pilot project in seven municipalities, it now operates in 26 locations across the state, serving 16 districts, 8 independent cities, 3 association municipalities, and one independent municipality.

The project targets very elderly people over 80 years old who still live in their own homes and do not yet require care. Community nurses, who serve as a crucial tool for local networking, provide consultation and support at home to delay the need for care. They act as a point of contact on a local level, initiating local health and independence-promoting social offers.

Community nurses strengthen existing structures in Rhineland-Palatinate by mediating access to existing, easily accessible, and suitable offers such as senior centers, exercise programs, or courses. They help combat loneliness among the very elderly by maintaining personal contact and building trustful relationships.

The project's goal is to provide a point of contact for the very elderly, addressing challenges of social isolation and loneliness in old age, and maintaining independence at home. Community nurses do not directly initiate care, but aim to delay care dependency. The project continues to expand its reach within Rhineland-Palatinate, with the aim to expand throughout the state by 2026.
