Kitchen whisk-induced orgasm may have inadvertently caused a fatal brain aneurysm in the individual concerned.

A 39-year-old woman in Belgrade, Serbia, was found dead in her apartment, marking a rare documented case of sudden death in women caused by a ruptured berry aneurysm triggered by anal self-stimulation.

According to forensic analysis, the woman's sexual excitement and subsequent orgasm may have temporarily caused her blood pressure to spike, leading to the rupture of a cerebral berry aneurysm and a fatal hemorrhage.

Berry aneurysms, small saccular aneurysms in the brain's blood vessels, are more common in women, especially postmenopausal women with low estrogen levels or individuals with hypertension. The physical exertion and orgasm involved in anal self-stimulation can elevate blood pressure enough to rupture such aneurysms, resulting in severe brain hemorrhage, disruption of oxygen supply, unconsciousness, and death.

The autopsy revealed a subarachnoid hemorrhage, bleeding in the space between the brain and a layer of tissue surrounding it, and a ruptured saccular aneurysm, a blood-filled sphere that ballooned from a blood vessel in the brain. The aneurysm measured 0.4 inches (11 millimeters) in diameter and had popped at the top of its dome.

An object protruding from the woman's bottom was identified as a spring twirl whisk, a kitchen tool, suggesting that she was using it for anal self-stimulation. The tool was inserted to a depth of about 4 inches (10 centimeters).

This case is unique as it is not a common cause of sudden death. Prior studies have linked only 0.15% of forensic autopsies (195 cases) to death during sex, with a majority of those cases being older males.

This rare phenomenon underscores the interplay between sexual physiology, vascular pathology, and forensic medicine. Forensic autopsies in such instances must consider sexual activity and orgasm as potential triggers when determining the cause of sudden neurological death, especially in women found alone under circumstances of sexual self-stimulation.
